29 CFR 1926.200(g)(1):     Construction areas were not posted with legible traffic signs at points of hazards:      a.  On or about 4/30/2012, North Bound Traffic Lanes, Traffic control signs were not posted where employees worked in the roadway to pour concrete for pipe footings and employees were exposed to struck-by hazards from vehicles.

29 CFR 1926.201(a):     Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers did not conform to Part VI of the Manual on Uniform Traffic Control Devices, (1988 Edition, Revision 3 or the Millennium Edition),      a.  On or about 4/30/2012, North Bound Traffic Lanes, Hand-signaling devices, such as stop/slow paddles were not used by an employee who stood in the traffic lane and directed traffic into an adjacent traffic lane which exposes employees to vehicular struck-by hazards.

29 CFR 1926.651(k)(1):     An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:      a.  On or about 4/30/2012, Trench East of Northbound Lanes, A competent person did not inspect the trench where an employee worked on pipe footings for a manhole which exposes employees to cave-in hazards.

29 CFR 1926.652(a)(1):  Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i):      a.  On or about 4/30/2012, Trench East of North Bound Lanes, The west and south trench walls were near vertical and adequate cave-in protection was not provided to employees who worked in the trench, which was greater than 5 feet in depth and was not classified for soil type.  Employees were exposed to cave-in hazards.
