29 CFR 1926.153(o): When damage to LP-Gas systems from vehicular traffic was a possibility, precautions against such damage were not taken: (a) Building# 5, East Side - on or about May 2, 2012 - two 500 gallon Liquid Propane Cylinders (LPG) were placed adjacent to the driveway without protection from vehicular traffic.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Front of Building# 4, Third Story Floor - on or about May 2, 2012 - employees were installing Tyvek Building Wrap on the balcony at an elevation of approximately 23 feet, 4 inches without any fall protection.
