29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a) Production Area, 150 Heller Place, Bellmawr, NJ 08031: On or about March 21, 2012, the employer did not provide employees authorized to perform Lockout/Tagout with locks to isolate machines and equipment from energy sources. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.217(c)(2)(i)(d): Point of operation guard(s) on mechanical power press(es) did not utilized fasteners not readily removable by the operator, so as to create minimize the possibility of misuse or removal of essential parts: a) Production Area, 150 Heller Place, Bellmawr, NJ 08031: On or about March 21, 2012, the employer used magnets to fasten plexiglass guards to mechanical power presses (Presses included, but were not limited to the Benchmaster 5 ton presses and the Kenco 5 ton presses). NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
