1926.453(b)(2)(v):     A body belt and a lanyard attached to the boom or basket when working from an aerial lift was not worn:  At a commercial building under construction, an aerial lift was being used by employees to access the work area without attaching to the boom or basket. Employees were exposed to fall hazards over 13 feet above the ground level.      At a commercial  building under construction, a Genie aerial lift, model S-45, was being used by employees without attaching a lanyard to the basket or boom at heights up to approximately 13 feet.
