OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees working from the monkey board, in case of an emergency are exposed to having a means of egress blocked: a) Monkey board, On or about 5/2/12 and at times prior thereto, in case of an emergency, employees were exposed to having a blocked means of egress by not having a geronimo line while hoisting or lowering pipe or rods into the well hole.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees working from the monkey board, in case of an emergency are exposed to a collapsing derrick: a) Rig Platform, On or about 5/2/12 and at times prior thereto, employees were exposed to being struck by a collapsing derrick that did not have 3 clamps per anchor line to secure the 1/2" diameter wire rope while sand pumping the well clean.
