29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) On or about March 7, 2012, at a job site located at 4305 Troost Ave. in Kansas City, MO, an employee working on the ground was not protected from falling objects by a hard hat. Other employees were working on a roof directly above him.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by a guardrail system, safety net system, personal fall arrest system, or a safety monitor system: (a) On or about March 7, 2012, at a job site located at 4305 Troost Avenue in Kansas City, MO, employees were exposed to fall hazards of greater than six feet while installing a temporary weather cover on a damaged roof. No type of fall protection system was in place or in use at the time of the inspection.
