29 CFR 1926.405(b)(1): Unused openings in cabinets, boxes, and fittings were not effectively closed:    a) Building West end of kitchen - On or about 3 May, 2012 - Employees who were in the kitchen area storing and retreiving tools, were exposed to an electrocution hazard when the main 240 volt panel box had 3 unused openings that were missing covers and the panel box door was left open. Employees were exposed to an electrocution hazard.

29 CFR 1926.405(a)(2)(ii)(J): Extension cord sets used with portable electric tools and appliances were not designed for hard or extra-hard usage:    a)Building West end storage area - On or about 3 May, 2012 - Employees were using an extension cord to power a hand planer that was not rated hard or extra-hard usage. Employees were exposed to an electric shock hazard.
