29 CFR 1926.304(i)(1): Each circular had-fed ripsaw was not guarded by a hood whcih completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut.    Jobsite: The circular hand fed rip saw that was used by employees to rip vertical wood siding was not guarded with a hood that completely enclosed the portion of the saw above the table.

Jobsite: The pump jack scaffold that the employee was working from at approximately 17 feet above the ground was not erected under the supervision of a person competent in scaffold erection.

29 CFR 1926.454(a):  The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    Jobsite: The employees who were working from the pump jack scaffolds without fall protection at heights of approximately 17 feet above the ground were not trained to recognize the hazards associated with the type of scaffold being used.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    Jobsite: The employees who were working from the pump jack scaffold at heights of approximately 17 feet above the ground were not protected from fall hazards by the use of guardrails or personal fall arrest systems.
