29 CFR 1926.251(e)(8):     Synthetic web slings were not immediately removed from service if any of the following conditions specified in 1926.251(e)(8)(i) through (e)(8)(v) were present:  (i) Acid or caustic burns; (ii) Melting or charring of any part of the sling surface; (iii) snags, punctures, tears, or cuts; (iv) broken or worn stitches; or (v) distortion of fittings.    At the worksite, on May 3, 2012, two nylon slings used to lift equipment had cuts, tears, and snags throughout.  This condition exposed employees to struck-by hazards.

29 CFR 1926.1416(d)(3)(i):     The employer did not ensure boom cranes manufactured after February 28, 1992 were equipped with a device which automatically prevents damage from contact between the load block, overhaul ball, or similar component, and the boom tip (or fixed upper block or similar component). The device(s) must prevent such damage at all points where two-blocking could occur:    At the worksite, on May 3, 2012, the Terex model number TC3063, S/N 45912992521 was used to lift air conditioning equipment.  The anti-two-blocking device was not functioning.
