29 CFR 1910.304(g)(5):     The paths to ground from circuits, equipment, and enclosures were not permanent, continuous, and effective:    a) On the 3rd floor, inside of the 50 pound packer area, there was a Weigh-Tronix check weight scale that was not grounded. The scale was plugged into an extension (flexible) cord and the extension (flexible) cord was plugged into a duplex outlet located near the packer area.    b) On the 7th floor, in the bulk processing area, there was a Patton heavy duty industrial air circulator fan with the ground pin missing from the plug of the electrical cord used to energize the fan.
