29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) 2075 Highway 97 Pleasanton, TX 78064: On May 1, 2012: On May 2, 2012 and at times prior there to, employee(s) were working on a 6 tiered fabricated frame scaffold at a height of over 36 feet. The scaffold had a means of access (ladder) to the 4th level but not to the 5th or 6th level. To access the 5th and 6th level, employee(s) would use the side frames of the fabricated frame scaffold exposing them to a fall of over 36 feet.
