29 CFR 1926.307(a)(2):  Vertical and inclined belts (paragraphs (e)(3) and (4) of this section) if not more than 2 1/2 inches (6.35 cm) wide and running at a speed of less than one thousand (1,000) feet per minute, and if free from metal lacings or fastening were not guarded with a nip-point belt and pulley guard.      On or about May 03, 2012, the employer did not ensure that the belt of the unidentified air compressor was guarded. The employees were exposed to an ingoing nip point hazard.

29 CFR 1926.307(d)(1):  Pulley(s) with part(s) seven feet (2.128 m) or less from the floor or work platform were not guarded in accordance with the requirements specified in paragraphs  (m) and  (o) of this section:  Note:  Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than 6 ft. 6 in. (1.976 m) from the floor or platform may be guarded with a disk covering the spokes.      On or about May 03, 2012,  the employer did not ensure that the pulley of the unidentified air compressor was guarded. The employees were exposed to an ingoing nip point hazard.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    On or about May 03, 2012,  employees who were installing roofing material were not provided fall protection. The employees were exposed to falls of 20 feet to the ground below.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    On or about May 03, 2012, employees were using an aluminum extension ladder with side rails that did not extend at least 3 feet above the edge of the roof.  The employees were exposed to falls of 20 feet to the ground below.
