29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: 1. In the machine shop, employees were using a Sears Craftsman Contractor Series 20 inch Industrial Rated Drill Press with no arbor (chuck) guard in place over the drill bit. 2. In the warehouse, employees were using a Powermatic Drill Press with no arbor (chuck) guard in place over the drill bit.

29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate. 1. In the machine shop, there was a balcony/cat walk used for storage located ten (10) feet above the floor with no load capacity (weight limit) sign in place to indicate how weight the platform can hold. 2. 29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: 1. In the machine shop on the west wall, employees were using a Baldor grinder with no tongue guards in place to cover both grinding wheels. The distance from the grinding wheels to the top of the eye shields was one and one half (1 1/2) inches.
