29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On 5/4/2012, the following machinery was not properly safeguarded at ingoing nip points.    a. The 709 Polar Mohr automatic bander barrier guard safety interlock switches were not in place to be operable.  b. The Stahl folder lines 717, 720 and 722 had barrier guards that were missing at the entry end on both sides.  The barrier guard at the 717 folder feeder rolls was broken and did not engage the safety interlock.  c. The Ty-Tech string wrapper at the 717 folding machine did not have barrier guarding at the backside.
