29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: 1. In the machine shop area, employees were using a Bridgeport Drill Press with no arbor (chuck) guard in place over the drill bit.

29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch: 1. In the production area, near the large bay door, employees were using a Maxx Air High Velocity 42 inch portable fan with a large unguarded opening (6" X 6") in the middle of the cover on the fan.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): 1. In the machine shop, on the west wall, there was a live 120 volt wire protruding out of conduit exposed to employee contact. There was no junction box or other electrical installation connected to the conduit.
