29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by moving parts: On or about May 1, 2012, and at times prior thereto, in the shear and press brake areas employees were allowed to operate various machines that were not properly guarded to prevent exposing to point-of-operation: a: A shear machine was provided with a guard. However, the guard was installed too close to the blade and the guard opening was big enough for a hand could enter the point-of-operation. b: A press brake machine operated via a foot pedal was found not having any guard provided. Employees were exposed to an amputation caused by the unguarded point-of-operation.
