29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and impalement hazards: 1. Inside of the fire suppression room, employees were exposed to impalement hazards from protruding pipe threads. There were four (4) horizontal pipe threads that extended out seven (7) inches each. There were three (3) verticle pipe threads that extended up nine (9) inches each. Employees go into the fire suppression room to check data and to do visual inspections of switches.
