29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) Trench located on Highway 97 east of Plestex Rd; Pleasanton, TX 78064: On May 3, 2012 and at times prior there to, employee(s) were working in an excavation that was 6 feet in depth. There was a ladder for employees to use that was approximately 40 feet from where the employees were working. The ladder was 2 pipe lengths away from where the employees were working. Each pipe length was 20 feet in length.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b)(2). a) Trench located on Highway 97 east of Plestex Rd; Pleasanton, TX 78064: On May 3, 2012 and at times prior there to, employee(s) were working in an excavation that was 6 feet in depth. The trench was not benched to the requirements of appendix b of this section. The initial bench was 2 foot wide when it needs to be 4 foot wide.
