On or about May 3, 2012, the loft and expander area were not kept clean from explosive dust accumulations. Dust accumulated on shelves, walls, ceiling structures, lighting and other equipment.

On or about May 3, 2012, one (1) cylinder containing oxygen was stored with one (1) cylinder containing acetylene. A chain was used to secure both of the cylinders to the building column.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, items A through D of this section: a) throughout the production area of General Partitions Mfg. Corp., 1702 Peninsula Drive in Erie, PA. On or about May 3, 2012, and times prior thereto, employees conducted maintenance and servicing work on machines that included, but were not limited to, the Wysong shearer and Verson press brake. The procedures were not detailed/workplace specific and did not meet the elements contained in 29 CFR 1910.147(c)(4)(ii)(B) through 29 CFR 1910.147(c)(4)(ii)(D). The procedures did not specifically detail items: (1) Specific methods for verifying isolation to ensure the machine is completely de-energized; (2) Each energy source and its magnitude; (3) Specifics on energy isolation points; (4) Methods to dissipate stored/residual energy; (5) Inspection of work area prior to restart; (6) Restart procedures. General Partitions Mfg. Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.147(c)(4)(ii), which was contained in OSHA inspection number 309338895, citation number 1, item number 2a and was affirmed as a final order on October 31, 2007, with respect to a workplace located at 1702 Peninsula Drive, Erie, PA 16505.

On or about May 3, 2012, the strain-relief of the flexible cord used to supply power to the automatic circular saw was damaged in that it was seperating from the cord and therefore the pull on the cord was no longer relieved by the strain-relief mechanism.
