29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:    a) Econolodge Addition Main Entrance - On or about 4 May, 2012 - Employees entering and exiting the building addition were exposed to an impalement hazard when the 12 inch long 1/2 inch diameter reinforcing steel bar protruding up from the concrete was not guarded. Employees were exposed to an impalement hazard.

29 CFR 1926.501(b)(1): Each employee working on, at, above, or near wall openings where the outside bottom edge of the wall opening is 6 feet or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches above the walking/working surface, shall be protected from falling by the use of a guardrail systems, a safety net system, or a personal fall arrest system.      a) Econolodge Addition Third Floor West Side - On or about 4 May, 2012 - Employees were doing cleanup and bringing in materials on the third floor of the new addition without any fall protection and were exposed to a fall hazard by an unguarded wall opening. Employees were exposed to a fall of 28 feet.

29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge:    a) Econolodge Addition Third Floor Landing - On or about 4 May, 2012 - Employees accessing the third floor of the addition were exposed to a fall hazard when the top guardrail was not installed to withstand a lateral force of at least 200 pounds. Employees were exposed to a fall hazard of 10 feet.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:    a) Econolodge Addition Stairwell from first to third floor - On or about 4 May, 2012 - Employees using the stairwell to travel between the third and first floors were exposed to a fall hazard because at least one handrail was not installed. Employees were exposed to a fall hazard of 10 feet between floors.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection.    a) Building Front - On or about 4 May, 2012 - Employees were framing out the extension walls and using an electric drill to drill holes for the anchor bolts. Employees were using an outlet that had a defective ground fault circuit interrupter. Employees were exposed to an electrocution hazard.
