29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b) Note: If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502. a) Building- On or about 1 May, 2012- During roof sheathing installation, employees were installing roof decking on a 12 on 12 pitched roof deck without utilizing conventional fall protection. Employees were exposed to a fall hazard of 10 feet. b) Building - On or about 1 May, 2012 - During roof sheathing operations employees were on the second floor of the building and were exposed to fall hazards while working near unguarded wall openings. Employees were exposed to a fall hazard of 12 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Building Right Side - On or about 1 May, 2012 - During entry and exit from the roof, employees were exposed to a fall hazard while using the ladder because the side rails were not extended at least 3 feet above the roof line. Employees were exposed to a fall hazard of 10 feet.

Employees were exposed to a fall hazard of 28 inches.
