29 CFR 1915.12(f):     Each employer whose employees work in confined and enclosed spaces or other dangerous atmospheres did not ensure all available information on the hazards, safety rules, and emergency procedures concerning those spaces and atmospheres was exchanged with any other employer whose employees may enter the same spaces:    In the water maker room, on the vessel Noble Sam Noble, located on the floating dry dock of AmFELS, LLC, employees were conducting service and testing on the twenty-six bottle carbon dioxide fire suppression system.  The employer failed to notify all employees working in adjacent rooms or areas of potential hazards including but not limited to release of carbon dioxide, oxygen deficient atmosphere and asphyxiation.

29 CFR 1915.89(l)(3)(ii):     The contract employer did not ensure the host employer knew about the lockout/tags-plus hazards associated with the contract employer's work and what the contract employer was doing to address these hazards:    In the water maker room on the vessel Noble Sam Noble, located on the floating dry dock of Keppel AmFELS, LLC, contract employees were assigned to service and test the twenty-six bottle carbon dioxide fire suppression system.  Prior to the servicing and maintenance on the system, Fire & Safety Specialists, Inc. did not inform the host employer that the fire suppression system had not been locked out and that the manual activation bar at the top of the two pilot cylinders of Carbon Dioxide had been removed disabling the time delay siren, alarm and pressure switches.

29 CFR 1915.89(l)(3)(i):     The contract employer did not follow the host employer's lockout/tags-plus program and procedures:    In the water maker room on the vessel Noble Sam Noble, located on the floating dry dock of AmFELS, LLC, employees were servicing and testing the twenty-six bottle carbon dioxide fire suppression system.  Fire & Safety Specialists, Inc. did not ensure its employees conducting the servicing and testing followed the host employer's lockout/tags plus program and procedures as outlined in the "Isolations" section of the Noble Safety Policy Manual.

In the water maker room on the vessel Noble Sam Noble, located on the floating dry dock of Keppel AmFELS, LLC, employees were assigned to service and test the twenty-six bottle carbon dioxide fire suppression system.  The employer did not ensure a means to isolate the system from the protected areas during the servicing and/or maintenance was provided and used.
