29 CFR 1910.134(f)(2): The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter.    At 2401 Hwy 14, Lake Charles, LA 70601; the employer did not ensure that each employee required to wear respiratory protection was properly fit-tested whenever a different respirator was used.  This condition was observed on or about May 2, 2012 where an employee, required to wear tight-fitting respirators when engaged in spray painting and sanding operations, was not fit-tested for the size, respirator model or make being worn.
