Public Health Service current at the time these evaluations and procedure took place:    a) In the workplace - employees cleaned up blood and other potentially infectious materials in the operating suite after surgeries.  The employer did not make available the Hepatitis B vaccination and vaccination series in accordance with current USPHS recommendations; on or about 5/4/12.    Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
