29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) On or about May 2, 2012, employees were not protected from head injury while working installing trusses in that, one employee was not wearing a hard hat while trusses were being boomed in from overhead.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) On or about May 2, 2012, employees were exposed to falls while using an extension ladder that was placed through a window opening and did not have the side rails extended at least 3 ft above the landing platform which was essentially a window sill. Employees also had to crawl through the opening while getting on and off of the ladder.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b): a) On or about May 2, 2012, employees were not protected from falls in excess of 6 ft while performing framing operations without the use of fall protection. Employees were working from exterior and interior top plates while installing trusses. Employees were also using trusses for support that were not adequately braced using temporary bracing methods. R.H. Framing was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent 29 CFR 1926.501(b)(13) which was contained in OSHA inspection 312136203 Citation 1 item 2a, issued on September 29, 2008 and became a final order on October 22, 2008 with respect to a worksite located at 13422 Crane Canyon, Colorado Springs, CO 80916.
