29 CFR 1910.23(a)(5): Every pit and trapdoor floor opening, infrequently used, was not guarded by a floor opening cover of standard strength and construction. While the cover is not in place, the pit or trap opening was not constantly attended by someone or protected on all exposed sides by removable standard railings. JOBSITE: The employee, working inside the pit, containing a submersible pump and used infrequently at a gas station, was not protected from the hazards of being hit and/or struck by vehicular traffic by the use of appropriate means. On or about 5/4/12, a vehicle drove over the "unprotected pit" when an employee was trying to get out of the pit, and the employee sustained an injury to the head.
