(a) On or about 05/05/2012 at 3 Northwind Court, Monroe, NJ 08831: The ladder provided did not extend 3 feet above the working surface of the roof to allow for safe access on and off the roof. Employees accessing this ladder were exposed to falls of approximately 19' to the ground below.
