The machine motor rotating shaft was not guarded.

On or about May 7 2012, and times prior thereto, in the food preparation area the power supply cord to MGE MTY meat grinder was spliced with a 12 inch cord. The employees were exposed to electrical shock hazards.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and cables were used as a substitute for the fixed wiring of a structure. On or about May 7 2012, and times prior thereto, employees operating a MGE MTY meat grinder and other kitchen appliances in the food preparation area used an extension cord to supply power to the appliances. The extension cord was run down a wall and under a table to supply power to the appliances.
