Elements missing and/or inadequate included:  - fit-testing procedures,  - procedures for proper use during routine and emergency situations,  - procedures for cleaning, storing, disinfecting, etc.  - training on proper use, donning and removing the respirator.    Condition observed on or about 5/4/2012.
