29 CFR 1910.23(c)(1):     Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides,    a) Grinding Room, at the conveyor on top of the ICA machine, on or about May 03, 2012 and at times prior to, employees stand and worked from a 40 inches X 48 inches wooden pallet that did not have guardrails, exposed to a 12 foot fall hazard. 29 CFR 1910.178(m)(5)(iii):     When the operator of an industrial truck is dismounted and within 25 ft. of the truck still in his view, the load engaging means shall be fully lowered, controls neutralized, and the brakes set to prevent movement.    a) Grinding room, on or about May 03, 2012 employees who were bagging peanut butter flour worked around forklift forks that were raised twelve feet above the floor while the operator was dismounted, exposed to a struck-by hazard. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.
