(a) On April 11, 2012, at a jobsite located at 7929 Sycamore Ave, Kansas City, MO., eight employees engaged in re-roofing activities on Multi-Family Apartment Complexes, were not protected from falling to lower levels. The employees were exposed to fall hazards of approx. 31 feet 5 inches to the ground level. Abatement certification for this violation is not required.

(a) On April 11, 2012, at a jobsite located at 7929 Sycamore Ave, Kansas City, MO., eight employees engaged in re-roofing activities on Multi-Family Apartment Complexes, had not been trained in the use and operation of required Fall Protection Systems. The employees were exposed to fall hazards of approx. 31 feet 5 inches to the ground level. Abatement certification is required for this item use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
