29 CFR 1910.147(c)(6): The employer did not conduct a periodic inspection of the energy control procedures at-least annually to ensure that the procedure and the requirements of this standard were being followed; and did not certify that periodic inspections of the energy control procedures had been performed: Workshop: LO/TO was used during routine gripper maintenance on the Mitsubishi 40" Press by an authorized employee on May 4, 2012 during a normally-staffed shift. Executive VP stated that he was in the process of review of the energy control procedures with Paychex who he had contracted to maintain the S&H programs; in addition, the employer had not certified that periodic inspection of the written procedures had been performed.

29 CFR 1910.147(c)(7)(i)(C): Employees whose work operations were in an area where energy control procedures were utilized were not instructed about the procedure nor the prohibition relating to attempts to restart or reenergize machines or equipment which are locked or tagged out: Workshop: Two temporary employees were not provided with training or instruction in the controlling of potentially hazardous and unexpected releases of energy sources (lockout/tagout) during servicing and maintenance of equipment, such as, but not limited to, the 240V shrinkwrapper (Shanklin), bindery equipment and presses. Imperial Graphics Communications, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(c)(7)(i) which was contained in OSHA inspection number 123192452, citation number 01, item number 002(c), issued on March 21, 2011 with respect to a workplace located at 600 Honeyspot Rd, Stratford, CT 06615 and became a final order on April 13, 2011.
