29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    On or about 04/27/12 at 1174 Bulldog Circle, Conyers, GA: Employees were exposed to fall hazards while performing steel erection work at a height of 28 feet above the ground.  The employer failed to ensure that steel erectors were provided with adequate fall protection at heights above 15 feet above the ground.

29 CFR 1926.752(b): Commencement of steel erection.  A steel erection contractor erected steel without obtaining written notification that the concrete in the footings, piers and walls or the mortar in the masonry piers and walls has attained, on the basis of an appropriate ASTM standard test method of field-cured samples, either 75 percent of the intended minimum compressive design strength or sufficient strength to support the loads imposed during steel erection.    On or about 04/27/12 at 1174 Bulldog Circle, Conyers, GA:  Employees were exposed to struck-by hazards while performing steel erection work at a construction site. The steel erection contractor failed to obtain a written approval to commence steel erection from the general contractor prior to erecting steel.
