29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a.  675 Park North Blvd. Suite 120, Clarkston, GA (Shop Area) -  Employees perform servicing and maintenance activities such as installing, setting up, adjusting, inspecting, repairing, cleaning and servicing a variety of machines and equipment (Packmatic and Heat Tunnel Machines).  The employer did not develop a lockout/tagout program consisting of machine specific lockout procedures and training was not provided to employees on the energy control measures, on or about 04/26/12.

There were no suitable eyewash/shower stations located in the immediate area for emergency use, on or about 04/26/12.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts and heated surfaces:    a.  675 Park North Blvd., Suite 120, Clarkston, GA (Line 2 Area) - Employees operate the Packmatic machine which wraps several bottles into one package.  Employees were exposed to rotating parts when walking and working in close proximity to the rotating parts.  The barrier guard and interlock switch was missing, on or about 04/26/12.     b.  675 Park North Blvd., Suite 120, Clarkston, GA (Line 2 Area) - Employees operate the Heat Tunnel machine which shrinks the plastic wrap around the product/bottles and is adjoined to the Packmatic machine.  Employees were exposed to thermal energy when walking and working in close proximity to the heating unit. The barrier guard and interlock switch was missing, on or about 04/26/12.

An employee was injured while working on the Packmatic machine on March 10, 2012, with lost and restriction workdays.  The incident was not recorded on the injuries and illnesses logs, on or about 04/26/12.
