(a) On, or at dates before March 19, 2012, employees in the south steel storage area were exposed to hazard when a C-hook attachment used in lifting operations involving a 12-ton Demag hoist was not marked to demonstrate its actual weight or the approved lift capacity of the attachment.  The hoist hook was overloaded as a result of the condition.
