29 CFR 1910.179(h)(4): Hooks. Hooks shall meet the manufacturer's recommendations and shall not be overloaded. (a) On, or at dates before March 19, 2012, employees in the south steel storage area were exposed to hazard when a C-hook attachment used in lifting operations involving a 12-ton Demag hoist was not marked to demonstrate its actual weight or the approved lift capacity of the attachment. The hoist hook was overloaded as a result of the condition. Abatement Note: Abatement certification is required for this item use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.179(n)(1): Size of load. The crane shall not be loaded beyond its rated load except for test purposes as provided in paragraph (k) of this section. (a) On, or at dates before March 19, 2012, employees in the south steel storage area were exposed to hazard when a 12-ton Demag hoist was used to lift a number of steel coils during receiving operations and the combined weight of a measuring scale, a C-hook lifting attachment, and the steel coils, exceeded the rated lift capacity of the hoist. Abatement Note: Abatement certification is required for this item use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
