29 CFR 1926.451(g)(1)(vii):     Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a) Outside, front of the building - on or about May 5, 2012, four employees using a personnel work platform were not provided with fall protection, exposing employees up to a 31-foot fall hazard. 29 CFR 1926.451(c)(2)(v):    Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:    a)  Outside, front of building, on or about May 2, 2012, an employee operating a JCB #752516 was observed being moved backward at the jobsite while an employee was elevated in an approved work platform, exposing the employee to ejection and struck-by hazards.    ABATEMENT CERTIFICATION AND DOCUMENTATION ARE NOT REQUIRED.

29 CFR 1926.602(c)(1)(vi):    Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.    a)  Outside, in front of the building, on or about May 2, 2012, the boom of a JCB forklift was left in the raised position with an employee working inside a work platform while the forklift was left unattended, exposing the employee to a fall/struck by hazards.    ABATEMENT CERTIFICATION IS NOT REQUIRED.

29 CFR 1926.760(a)(1):     Each employee engaged in a steel erection activity that was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    a)  North side of the building, on the roof on or about May 02, 2012, guardrail, safety nets or personal fall arrest systems were not provided for employees walking/working close to the edge of the roof, exposing them up to a 31 foot fall hazard.    ABATEMENT CERTIFICATION AND DOCUMENTATION ARE NOT REQUIRED.
