29 CFR 1910.67(c)(2)(ii): Employees were not trained to operate an aerial lift: (a) 1030 Pointview Avenue, Ephrata, Parking Lot - On or about May 7, 2012 an employee was loading a Terex aerial lift onto a lowboy trailer which then overturned causing serious head injuries and the employee was not trained to operate the lift. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

(a) 1030 Pointview Avenue, Ephrata, Parking Lot - On or about May 7, 2012 an employee was loading a Terex aerial lift onto a lowboy trailer and was not tied off to the basket anchor point. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
