29 CFR 1926.601(b)(14): All vehicles in use were not checked at the beginning of each shift to assure that the following parts, equipment, and accessories were in safe operating condition and free of apparent damage that could cause failure while in use: service brakes, including trailer brake connections; parking system (hand brake); emergency stopping system (brakes); tires; horn; steering mechanism; coupling devices; seat belts; operating controls; and safety devices. All defects were not corrected before the vehicle is placed in service. These requirements also apply to equipment such as lights, reflectors, windshield wipers, defrosters, fire extinguishers, etc., where such equipment is necessary: a) North of Doyle Drive on or about May 8, 2012, and at times prior thereto, a ? inch diameter bolt was used to secure a 2 foot long by 3-5/8s diameter pin in the boom of a Caterpillar 350 L excavator, Ferma number 2. The bolt was broken off and bent at the threads, and there was no nut on the bolt. The pin secured a portion of the muncher to the excavator's boom. The excavator was used to separate rebar from the concrete on the recently demolished Doyle Drive approach to the Golden Gate Bridge. Employees that sprayed water from hoses onto the demolished materials were exposed to the hazard being caught in the jaws of the cruncher. (b) North of Doyle Drive on or about May 8, 2012, and at times prior thereto, a ? inch diameter piece of rebar was used to secure a pin to the boom of a Caterpillar 345 excavator, Ferma number 13. The rebar was looped through the pin's hole. The pin was used to secure a jack hammer the excavator's boom. The jack hammer was used to separate concrete on the recently demolished approach to the Golden Gate Bridge. Employees that sprayed water from hoses onto the demolished materials were exposed to the hazard being struck by the jack hammer.
