29 CFR 1910.147(c)(4):     Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    On or about 04/19/12 at Macon, GA: Employees were exposed to caught between hazards while maintaining and servicing a Selco Model V6 vertical baler at a supermarket. The employer failed to develop a lockout tagout procedure for the baler.

29 CFR 1910.147(c)(7)(i):     The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:    On or about 04/19/12 at Macon, GA: Employees were exposed to caught between hazards while performing baling operations at a supermarket.  The employer failed to provide training to those employees participating in operations requiring lockout tagout operations to include tie slot cleaning.

29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about 04/19/12 at 1425 Rocky Creek Rd., Macon, GA: Employees were exposed to caught between hazards while operating a baler at a supermarket.  The employer failed to ensure that barrier guard on the baler was properly maintained.  Sections of the baler's front barrier guard were damaged and missing.
