On or about May 8, 2012 - at the above addressed jobsite, an employee was performing siding installation from a pump jack scaffold, twelve (12) feet eight (8) inches above the ground, that was erected on unstable objects, such as a cinder blocks, thereby exposing the employee to a fall.

29 CFR 1926.451(f)(6): The clearance between scaffolds and power lines shall be as follows: Scaffolds shall not be erected, used, dismantled, altered, or moved such that they or any conductive material handled on them might come closer to exposed and energized power lines than as follows: a. On or about May 8, 2012 - at the above addressed jobsite, employees were required to erect, work from and dismantle a pump jack scaffold that was located less than three (3) feet from power lines, thereby exposing employees to electrical shocks.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. a. On or about May 8, 2012, at the above addressed jobsite, an employee performing siding installation from a pump jack scaffold, approximately twelve (12) feet eight (8) inches above the ground, was exposed to injuries from falls due to the lack of fall protection.
