29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a.  1154 Highway 59, Commerce, GA  - Employees were exposed to unexpected start up energy.  The employer did not develop machine specific procedures for the multi-energy source machines such as, but not limited to the Metal Shears, Hydraulic Press Brakes, CNC/VMC Mill, and CNC Lathe, on or about 05/01/12.

29 CFR 1910.242(b):  Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:    a.     1154 Highway 59, Commerce, GA (Throughout Shop Area) - Employees were exposed to flying debris hazard.  Employees are using compressed air to clean the metal/steel shavings off the machines and employees clothing.  The air pressure gauge measured greater than 30 psi.
