29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered. a) Weldship Corporation, Jumbo Tube Room Paint Booth. - On or about May 8, 2012, employees were not protected from falls 10 feet 1 inch above ground level through 50 and 1/2 inches long by 14 and 1/2 inches wide holes created in the paint booth roof by the removal of light fixtures and Plexiglas lenses. b) Weldship Corporation, Jumbo Tube Room Paint Booth. - On or about May 8, 2012, employees were not protected from falls 10 feet 7 inches above ground level around the exterior edges of the paint booth roof.
