29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    On or about May 9, 2012, the employer did not ensure employees were protected from a fall while working from a ladder jack scaffold. Employees were exposed to a 20 fall to the level below while roofing duties from the scaffolding, without any standard fall protective system/devices.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    On or about May 9, 2012, while performing roofing work to include putting new weatherproofing on a residential house,  the employer failed to protect employees from a fall of approximately 21 feet 11 inches from a 12/12 pitch roof.
