29 CFR 1910.1030(c)(1)(i):     The employer having employee(s) with occupational exposure did not establish a written Exposure Control Plan designed to eliminate or minimize employee exposure:    (a) Kennesaw Pediatrics- The employer had not established a written exposure control plan for employees performing and assisting with medical procedures such as, but not limited to, injections. The clinics written exposure control was a Laboratory Safety plan. Deficiencies include, but are not limited to, no exposure determination, no review and update at least annually to document changes in technology (such as safe sharp devices), no documentation of input from non-managerial employees regarding the identification, evaluation, and selection of safe sharp devices, and specifically how this facility plans to comply with paragraphs (d) through (h) of the bloodborne pathogens standard.    ABATEMENT DOCUMENTATION AND CERTIFICATION REQUIRED

29 CFR 1910.1200(e)(1):     The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    (a) Kennesaw Pediatrics- The employer had not established or implemented a hazard communication program to include a written program, maintenance of material safety data sheets (MSDSs), and training. Employees are exposed to hazardous substances such as, but not limited to, Wavicide containing glutaraldehyde, PSS disinfecting wipes containing isopropanol and benzyl ammonium chloride, and Lysol containing quaternary ammonium compounds.    ABATEMENT DOCUMENTATION AND CERTIFICATION REQUIRED
