29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    On or about 05/01/12 at Milledgeville, GA: Two employees were exposed to fall hazards while performing stucco work from an aerial lift.  The employer failed to enforce the proper use of fall restraint equipment for employees occupying the aerial lift.  One employee's lanyard was not tied off to the aerial lift.  The second employee's lanyard was attached to the top rail, in lieu of the aerial lift's anchor point.
