29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502.    An employee without fall protection was performing roofing activities at heights over six feet above the lower level.    *Abatement documentation is required per 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1):     Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    Ladders used to access upper levels were not extended at least 3 feet above that level nor were they secured to the surface level being accessed.  These included:    a)  The straight ladder used to access the ladder jack scaffold on the right side of the house,    b)  The straight ladder used to access the left side of the porch roof, and    c)   The straight ladder used to access the ladder jack scaffold on the front porch roof, leaning against the peak of the gable of the porch on the front of the house.     *Abatement certification is required per 29 CFR 1903.19(c).

29 CFR 1926.451(g)(1)(i): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level.  Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    Employees were working without fall protection from a ladder jack scaffold over ten feet above the lower level.    Seamless Gutters Unlimited, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.451(g)(1)(i), which was contained in OSHA inspection number 310766746, Citation number 01, Item 01, and was affirmed as a final order on 8/28/08, with respect to a workplace located at 819 3rd Street, New Glarus, WI.    *Abatement documentation is required per 29 CFR 1903.19(d).
