Listed, labeled, or certified equipment shall be installed and used in accordance with instructions included in the listing, labeling, or certification: a) On or about April 6 & 13th, 2012, at the jobsite located at 4126 S. Calumet Avenue, Chicago, IL. - Employees performing drywall activities in a rehab were using a metal electrical power box that was designed to be attached to the wall to power electrical hand tools, such as but not limited to saws and drills, thereby exposing the employees to electrical hazards.

29 CFR 1926.403(I)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means: (A) by location in a room, vault, or similar enclosure that is accessible only to qualified person; (B) by partitions or screens so arranged that only qualified person will have access to the space within reach of the live parts; (C) by location on a balcony, gallery, or platform so elevated and arranged as to exclude unqualified persons; (D) by elevation of 8 feet or more above the floor or other working surface and so installed as to exclude unqualified persons: a) On or about April 6 & 13th, 2012, at the jobsite located at 4126 S. Calumet Avenue, Chicago, IL. - Employees performing drywall activities in a rehab were using electrical receptacles/outlets with exposed live parts to power electrical hand tools, such as but not limited to saws and drills, thereby exposing the employees to electrical hazards.

Calumet Avenue, Chicago, IL. - Employees performing drywall activities in a rehab were using a staircase that was not equipped with handrails, thereby exposing the employees to fall hazards of approximately fifteen (15) feet.
