29 CFR 1910.1048(d)(1)(i): Employees of a workplace covered by this standard were not monitored to determine their exposure to formaldehyde: Location: At or near 1551 W. Wakefield Ave., Sikeston, MO 63801. The employer did not determine employee exposures to formaldehyde when workers used Hydrochem product code A196, Corrosion Inhibitor A196 which contains < 5% formaldehyde. This product was used during the boiler descaling operation.

29 CFR 1910.1200(h)(1): Employers did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees had not previously been trained about was introduced into their work area and chemical-specific information was not always available through labels and material safety data sheets Location: At the Sikeston, Missouri power plant at or near 1551 W. Wakefield Ave., Sikeston, Missouri, 63801. The employer did not ensure that all employees were aware of the following hazards when performing a boiler clean-out operation as follows: 1.) The potential for generation of hydrogen gas when 31.5% hydrochloric acid was stored in a carbon steel tank. 2.) The need to ensure proper venting of the carbon steel tank when used to store 31.5% hydrochloric acid.
