Location: At the retirement facility at or near 2825 Bloomfield Road, Cape Girardeau, MO 63703. On or about May 10, 2012, workers were performing roofing activities on a roof with a pitch of approximately 4:12 without any protection from falls when working at a height of greater than 6 ft.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Location: At the retirement facility at or near 2825 Bloomfield Road, Cape Girardeau, MO 63703. The employer did not ensure that the ladder extended a minimum of 3 ft. above the roof. The ladder was used by the workers to access the roof.
