29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: Employees are exposed to fall hazards from a two story home. An extension ladder to access work on the roof did not extend three feet above the upper landing surface nor were there any grasping device, such as a grab rail to assist employees in mounting and dismounting the ladder.

Employees were engaged in roofing activities working from in six in twelve pitch roof, employee was exposed to falls of approximately 10 feet from the eave to the ground. Employees were not protected against the hazard of falling.
