29 CFR 1926.102(a)(1):     Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    Employees using pneumatic nail guns shingling were exposed to eye injuries, employees were working without eye protection.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    Employees are exposed to fall hazards from a two story home. An extension ladder to access work on the roof did not extend three feet above the upper landing surface nor were there any grasping device, such as a grab rail to assist employees in mounting and dismounting the ladder.

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502.    Employees were engaged in roofing activities working from in six in twelve pitch roof, employee was exposed to falls of approximately 10 feet from the eave to the ground. Employees were not protected against the hazard of falling.    Hernandez Martinez Construction was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard (29 CFR 1926.501(b)(13), which was in OSHA inspection number 313012049, citation number 1, item number 2a, issued on 06-18-2009, and was affirmed as a final order on 07-23-09 with respects to a workplace located at 2335 Pontoon Rd., Granite City, IL 62040    Hernandez Martinez Construction was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard (29 CFR 1926.501(b)(13), which was in OSHA inspection number 313017386, citation number 1, item number 2, issued on 12-30-2010, and was affirmed as a final order on 01-27-2011 with respects to a workplace located at 116 Chamberlain Edwardsville, IL 62025    Hernandez Martinez Construction was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard (29 CFR 1926.501(b)(13), which was in OSHA inspection number 313017477, citation number 1, item number 2, issued on 12-30-2010, and was affirmed as a final order on 01-27-2011 with respects to a workplace located at 3415 Vicksburg Dr. Edwardsville, IL 62025
